General Summary

The Prenatal Care Coordinator is responsible for screening and assessing pregnant women  to determine what services women are eligible for utilizing. The program also strives to successfully link all pregnant women with a Community Health Worker, and prenatal health education services. The position is accountable for successfully implementing the Infant and Maternal Grant’s goals and desired outcomes.

Primary Job Responsibilities:

  • Completes screenings and assessments.
  • Facilitates access to prenatal care, connection to resources, and a Community Health Worker.
  • Provide education based on client needs. 
  • Collaborates with agency partners to understand and provide services to patients and their families. 
  • Provides follow up with patients on all  services and referrals made on their behalf. 
  • Assist with the completion and submit mission of accurate and timely programmatic forms, reports and administrative paperwork, including electronic databases. 
  • Assists administration for the grants process through assessing, tracking, payment/check requests, and documentation
  • Provides technical assistance to participants.
  • Coordinates classes with facilitators.
  • Organize events for participants.
  • Attends staff meetings, department training/meetings, etc.
  • Create and maintain creative materials and announcements.
  • Maintains the online nonprofit database for enrolled participant’s classes.

Conditions of Employment

  • Possession of a valid Ohio  driver’s license is a requirement for this job.
  • Must pass a pre-employment criminal, drug, and tobacco free background check.
  • Successful candidates must submit a pre-placement medical clearance.
  • Fingerprinting, and subsequent clearance, may be required for this position depending on assignment.

Experience Required:  

Minimum of one of experience working in a social service setting preferable with pregnant women

Educational Requirements:  

Bachelors welcomed but not required, experience in Social Work, Psychology, Sociology, Counseling or Education.

Fulltime/$17 per hour
