Outreach Coordinator

Outreach Coordinator (Full-time Permanent):

a Full-Time Outreach Coordinator is needed to assist clients with basic needs on an ongoing basis. Advocates for clients in the community. Assist client with completing applications in-house and in carrying out their goals in the field. Assist clients in linking and accessing mental health and substance addiction services through education and advocacy. Stabilizes new clients by familiarizing them with the new environment. Performs intake. Captures HMIS information. Monitors and intervene with clients in crisis situations to defuse inappropriate client behaviors as required.

Position Requirements:

An Associates’ Degree in social services or related field is required. A Bachelor’s Degree is preferred. Prior experience working with individuals with severe and persistent mental illness and/or substance misuse is preferred. Positive attitude is a must.

Two (2) years of prior work experience is preferred. The successful candidate must have strong oral and written communication skills and be proficient with using and understanding computers (Word and Excel) and other basic technology. Driver’s License is required.
