Pathways Specialist – Queens Village Program

Hours                                                   Full-time; 8a-5p, flexible, remote, live within Toledo area

FLSA Status                                         Non-Exempt; hourly position

 

POSITION QUALIFICATIONS:

  • A Bachelor’s degree in health-related field or equivalent degree required
  • Previous experience in public health or a hospital setting desired
  • Knowledge of project evaluation, statistics, and medical terminology helpful
  • Experience with project management desired
  • Ability to work with diverse constituents and coalition development experience required
  • Capacity to advocate for and possess cultural competency around black families and communities
  • Strong sense of connection, compassion, concern for the well-being and success of black women
  • Healthcare and grants management experience desired
  • Proficient in the use of Microsoft Office Suite including Word, Excel, PowerPoint and Teams
  • Ability to learn commonly used software programs as well as specialized software
  • Excellent written, verbal and presentation skills
  • Experience working within the Pathways Community HUB Model desired
  • Critical thinker, able to research, design, oversee and complete projects with minimal guidance
  • Self-starter, independent worker, with ability to work as a member of a team desired
  • Must be adaptable and willing to accommodate routine travel within Ohio and elsewhere as required.

 

JOB DUTIES & RESPONSIBILITIES

  • Help connect and build bridges between Queens Village community programs and community members
  • Build awareness, engagement and participation of moms and families to lead community work
  • Lead planning for community engagement and relationship building
  • Maintain relationships with community members and stakeholders
  • Execute a social media strategy
  • Lead community events, neighborhood meetings, activities, and canvassing
  • Collect information and evaluation on community programs and events
  • Assist with day-to-day operations of the Pathways HUB
  • Provide technical assistance to Community Health Workers, agencies, and staff
  • Assist in quality improvement processes and implementation for HUB operations
  • Lead special projects that lead to improved outcomes among clients
  • Other duties as assigned
