Case Management Coordinator

Seeking motivated/positive individual to serve as a senior case manager within a women’s housing program. Knowledge of homelessness and its causes a necessity. Familiarity of community resources and programming a must. Some supervisory experience helpful.  This is a FT position. Benefits provided.

SUMMARY

The case manager, in utilizing Stabilization Components of a client’s case plan, is key to empowering women in setting goals that will transition them from dysfunction to self-sufficiency and independence.

ESSENTIAL RESPONSIBILITIES

  • Assist new clients in acclimating to transitional housing environment
  • Provide support and case management services for clients who are residing in the community – Aftercare
  • Assist clients in developing Individualized Case Plan based on Stabilization Components
  • Meets regularly in individual sessions with clients to document case plan requirements
  • One-on-one sessions should include discussion and assignments related to issues such as self esteem, stress management, relationship building, budgeting, education and job training, crisis intervention, exit interviews and development of aftercare plan
  • Maintains confidential records on each assigned client, including census reports, meeting attendance, etc. Documents client progress
  • Coordinates services among linked agencies and services
  • Attends meetings with drug treatment team, LCCS, probation, municipal and family court, and mental health agencies as necessary
  • Attends weekly case management meetings to report client progress
  • Attends client house meetings
  • Available for crisis intervention, transportation, and consultation as needed
  • Provides data and completes all reports required by grants, for public relations purposes, and agency
  • Oversight and responsibility of small case staff
  • Demonstrates a passion for the agency’s mission and values
  • Performs other related duties as assigned

KNOWLEDGE, ABILITIES & SKILLS
• Bachelor’s Degree or equivalent in social services field
• Knowledge of homelessness, addiction, domestic violence and mental illness beneficial
• Excellent communication skills – both written and oral
• Requires excellent organizational skills
• Team oriented
• Ability to work with diverse populations
• Must be computer literate – proficient in Microsoft Word and Outlook
• Must be able to work as part of a team, as well as individually
• Must be able to organize and prioritize work, be proactive, take initiative, resolve complex issues, follow through, and simultaneously manage multiple priorities
• Must maintain a high level of confidentiality, working with sensitive and privileged information
• Criminal background check, covid-19, drug and TB testing prior to employment

Job Type: Full-time

Pay: $37,500.00 – $39,000.00 per year starting
